The diagnosis is clinical in front of an irregular nail hypertrophy, most often large toe. A severe congenital onychogryphosis affecting the twenty nail beds has been recorded in two families who present the dominant allele for a certain gene [ 6 ] , [ 7 ] . The congenital onychogryphosis of the fifth toe (the small toe) is quite common, but asymptomatic and rarely brought to the attention of health professionals. On the contrary, it is brought to the attention of manicures which regularly lighten the griffus nail flat. Untreated, onychogryphosis can be a source of cosmetic difficulties (put on or cut your nails), pain or infectious complications [ 4 ] .
